Entering Readings by Hand and Viewing Past Readings

Now that remote health monitoring is part of your care plan, the NYU Langone Health app will remind you to take readings and do other tasks.

If your health monitoring device does not let you automatically sync readings to your smartphone, you can still enter readings by hand (manually) into the NYU Langone Health app.

Follow these steps to enter your readings by hand.

Note: When possible, we strongly recommend syncing your health monitoring device with your smartphone and the NYU Langone Health app. This produces the most reliable results.

View Assigned Tasks in "To Do"

1. Open the NYU Langone Health app on your smartphone.

2. Sign in with your NYU Langone Health MyChart username and password.

3. After signing in, you will see the home screen of the NYU Langone Health app.

4. In To Do, you can see tasks you need to do for your care plan.

This example shows a task for blood pressure monitoring. You may have other types of tasks. Your tasks depend on the type of remote health monitoring program you have.

Complete Flowsheet Tasks by Entering Readings by Hand

You will complete flowsheet tasks to gather important vitals and other information. This helps you and your provider track your health over time.

1. In the NYU Langone Health app, click on To Do to see your assigned tasks. The task in this example is telling you to "Record Your Blood Pressure" today.

2. Track My Health will open.

To enter a reading by hand (manually), select Add readings.

3. Check that the date is correct. Then select the blank field.

4. Now you can use your smartphone's key pad to enter the reading from your health monitoring device.

5. When you are done filling in the reading, select Save. This captures your manually entered health data.

6. You will go back to the Track My Health screen. Here you will see your most recent reading, the one you entered by hand.

7. Select To Do in the top left corner of the screen to return to your list of assigned tasks.

8. You will return to the To Do screen.

The blood pressure task is now completed.

If you do not have any more tasks to complete, you will see a friendly acknowledgement of your accomplishment.

9. Select the back arrow in the top left of the screen to return to the home screen of the NYU Langone Health app.

Note: When possible, we strongly recommend syncing your health monitoring device with your smartphone, including Apple Health (or Google Fit for Android) and the NYU Langone Health app. This produces the most reliable results.

View Past Readings in Track My Health

In addition to completing tasks, you can also use Track My Health to view past readings and see trends in your health data over time.

1. From the home screen of the NYU Langone Health app, scroll down to the Track My Health icon.

2. Track My Health will open. Here you will see your past readings.

3. You can also review your past readings by selecting the different time intervals (for example, Day, Week, Month, or Year) at the top of the screen.
